# Postpartum Depression

Not the racing version. The flat one. The one where you are doing everything correctly and feeling almost nothing while you do it.

You feed her, you change her, you get up in the night, you answer the messages asking how you are. And underneath all of it there is a strange absence where the feeling was supposed to be, and you have not told anyone about it because saying it out loud sounds like an accusation against yourself.

## What It Actually Feels Like from Inside

Most women describe it as distance rather than sadness. You are behind glass. You can see your family on the other side of it, you know they are yours, and you cannot reach them. Some women cry constantly. A great many do not cry at all, which is one of the reasons this gets missed for so long.

    Losing interest in things that were reliably yours before, and not minding that you lost them

You did not fail to love her. Something is standing between you and the feeling.

## This Is Not the Same As Postpartum Anxiety

A lot of women arrive not knowing which one they have, so it is worth setting them side by side. Postpartum anxiety is the alarm system stuck on: checking her breathing, waking at every sound, intrusive images that horrify you, a body that will not stand down. It is loud and it is exhausting and it is happening in the direction of the baby.

Depression runs the other way. It is quiet, and it moves away from things rather than toward them. It is flatness, distance, a kind of muffled numbness, and a heavy sense that you are getting this wrong in a way that other mothers are not. Anxiety says something terrible might happen. Depression says something already has, and it is me.

Plenty of women have both, in layers, and they can trade places month to month. Knowing which one is loudest today changes where we start, so we work that out together rather than deciding it from a label. If the description on the anxiety page fits you better, read that one instead.

## It Is Not the Baby Blues

The first week or two after a birth is weepy and raw and enormous for almost everybody. Hormones drop, sleep vanishes, and everything feels amplified. That usually lifts on its own within a couple of weeks and it does not take your personality with it.

This is different in duration and in depth. It stays. It flattens things rather than heightening them. And it does not respond to the reasonable arguments you keep making at yourself about how much you have to be grateful for.

## It Can Start Months After the Birth

This surprises people, and it is one of the main reasons women dismiss what is happening to them. You were fine at six weeks, so this cannot be that. But it can begin at four months, at seven, when you go back to work, when you stop feeding, when the visitors stop coming and the house goes quiet and the actual size of the change finally lands.

Arriving late does not make it less real and it does not put it outside the window for help.

## It Happens to Women Who Wanted the Baby Very Much

Including women who waited years for her. Including women who did treatment, or who had a loss before this pregnancy, or who spent a decade certain this was what they wanted. Wanting the baby does not protect you, and for those women the guilt tends to be worse, because the gap between how you expected to feel and how you actually feel is that much wider.

It is also not a character failure and not a verdict on the kind of mother you are. It is a condition, and it has an enormous amount to do with hormones, sleep, an altered body, an identity that changed overnight, and often a birth that took more from you than anyone acknowledged.

## How We Would Work with It

Slowly, and without asking you to perform enthusiasm. Talk therapy has a place here, and so does Internal Family Systems, which gives you a way to approach the part of you that has gone quiet rather than shouting at it to come back. Somatic work matters because depression is physical before it is verbal, and the body is often where the first movement shows up.

Where a difficult birth sits underneath the flatness, and it frequently does, we work with that too rather than treating the mood in isolation.
