# Birth Trauma Therapy for Charlottesville

Charlottesville is an academic and medical town, and it attracts people who are very good at achieving under pressure and who mostly moved here for the work. Both of those facts shape what happens after a difficult birth.

## Achievement Culture Meets a Body That Will Not Perform

Faculty, researchers, residents, graduate students, the professional staff around all of it. What that population has in common is a lifetime of meeting standards through preparation and effort. It has almost always worked. Then comes a birth, which cannot be prepared for in that way and does not reward effort, and afterwards a recovery that refuses to follow a schedule.

The response is usually to try harder. Read more. Optimize the sleep. Return to the lab or the ward at the earliest possible date to prove that nothing has changed. Underneath that there is often a specific and rarely spoken fear: that the birth revealed something about you, that you were not composed, that you lost control in front of people, and that this is a truer picture of you than the competent one.

It is not. But that belief is remarkably durable in people whose self-respect is built on performance, and it does not respond to reassurance. It responds to actually going back to the moment with enough support that it stops being evidence.

## No Family Within Reach

Most people here came for a position. Family is elsewhere, sometimes far. The community is warm but it is also transient, so the friend who would have been your person is on a two year contract and then gone.

That has a particular effect on new motherhood. The support you have is largely institutional: colleagues, a department, a program. Institutions are not built to hold you at three in the morning. And there is a reluctance to be the person in the department who is visibly struggling, especially if you are pre-tenure, in training, or on any kind of clock where being seen as fragile carries a cost.

## The Clock Makes People Wait

If your appointment, your program or your funding has an end date, everything gets deferred to after it. After the defense. After the match. After review. The birth goes in the queue behind all of those, and the queue never empties.

This is the argument for an intensive rather than weekly work for some patients here. A concentrated block, with a ninety minute intake before and a thirty minute close after, fits a life organized around terms and rotations better than a standing hour that will collide with a conference in three weeks.

## Practical

I see Charlottesville patients by secure video. I am private pay only, which for people in small professional communities also means nothing goes to an insurer under your name. A superbill is available on request and reimbursement is not something I can guarantee.

I am licensed in Virginia only, so you need to be physically in Virginia during sessions, including over a summer spent working elsewhere.

## What the Work Is Not

It is not a course you can be good at. There is no reading list to get ahead on and no version of this where preparation earns you a faster result. For a lot of patients here that is the genuinely uncomfortable part, and it is also the part that turns out to matter.

It is also not a demand that you fall apart. Going near a frightening memory with support is not the same as being overwhelmed by it, and a great deal of what I do is regulating the pace so that you stay present rather than flooded.
