It sets out the services we have discussed and the expected charges for them, so you know the numbers before you decide anything. It is an estimate rather than a contract, and actual charges can differ if the course of care changes. You can ask me for one at any point, and I will put it in writing. ## Disputing a Bill If you receive a bill that is at least 400 dollars more than your Good Faith Estimate, you have the right to dispute that bill. You may find more information about your rights under the No Surprises Act, and how to initiate a dispute, at www.cms.gov/nosurprises. ## Requesting an Estimate You are entitled to a Good Faith Estimate whether or not you request one. If you would like one in writing ahead of scheduling, you can ask by phone at 804-395-5362 or through the contact page, and it will be provided before your first appointment.
